Age 75 - Defined Benefit to Accumulation account transfer form (303KB) Complete this form if you are about to reach age 75 and hold a Defined Benefit account, to transfer your benefit to an Accumulation account.
Authority to access and release medical details form - death claims (248KB) Allow QSuper to access details of a member's medical records in order to assess a death benefit claim.
Authority to release information form (77KB) Give QSuper the authority to release information on your superannuation account/s to an individual or organisation.
Change of details form - Allocated Pension account (73KB) Advise QSuper to change your pension payment amount, or to advise us of a change to your banking details.
Claim form - Accumulation account (100KB) Instruct QSuper to transfer your benefit to a QSuper Allocated Pension account, make a cash withdrawal (if you have non-preserved money available), or transfer your benefit to another complying superannuation fund (if you are no longer employed by the Queensland Government).
Claim form - Allocated Pension account (81KB) Make either a full or partial withdrawal from your Allocated Pension account.
Claim form - Defined Benefit account (163KB) Transfer your benefit to either a QSuper Accumulation or QSuper Allocated Pension account, make a cash withdrawal (if you have unrestricted non-preserved funds available), or transfer your benefit to another complying superannuation fund (if you have stopped working for the Queensland Government).
Claim form - State and Police account (156KB) Withdraw your benefit because you have left your job.
Claim form - State and Police account preservation (94KB) Claim the maturity benefit of your previously preserved amount (must be 55 or older), or cancel a preservation arrangement that you have previously made (aged under 55).
Claim on spouse's benefit (325KB) Claim a benefit following your spouse's death.
Contribution splitting form (395KB) Split all or part of your eligible contributions into your spouse's QSuper account from your Accumulation account.
Defined Benefit account disability claim form (571KB) Make a claim if you have a Defined Benefit account and your claim for disability insurance benefits has been approved.
Defined Benefit partial claim form (429KB) Withdraw unrestricted money from your Defined Benefit account if you are currently employed by the Queeensland Government.
Departing temporary resident claim fact sheet & form (405KB) Claim your Australian superannuation benefits, if you entered Australia on an eligible temporary resident visa but have now permanently departed.
Deposit form (115KB) Make a one-off voluntary contribution to your QSuper account.
